How to add a checkbox in Word 2003/2007
Word 2003:
View > Toolbars > Forms
Word 2007:
Click the Office 2007 button (top left of screen).
Click Word Options
under Popular - select to show the developer tab in the ribbon
Choose Legacy Tools on the Developer Tab
Check Box form field is listed there under Legacy Forms.
