This is common in upgrades from earlier revision of prevail, its more common in docs that were converted from old to new merge.

You must open the document in the document designer and remove the merge association toolbar.

Once removed it should fix all docs but this may be a per doc fix.

Click options on the toolbar and remove and then click yes or continue at the prompt.
