Before you request a report, please visit the Prevail Report Vault to determine whether or not there is already a report available that suits your needs. If you don't find what you're looking for, you can use this form to request a new report or a modification of an existing report. The cost of custom reports is $25.00 per report, payable via check or credit card. You will be contacted by phone for payment information upon approval of this report request. If you are at the Premium Support level, you are entitled to five (5) free custom reports. If you are unsure about your support level or the number of free reports you have remaining, please give us a call at 407.228.4400 or email reports@prevail.net. Reports are usually delivered within 7 to 10 business days from receipt of payment. Completed reports are delivered via email to the address entered in the form below.
